Camp Info & FAQs
How do I upload my athlete’s physical exam certification?
All campers are required to have a current physical exam and proof of medical insurance on file prior to attending camp.
These documents must be uploaded to The Good Game App before check-in at camp.
Required Uploads:
Updated Physical Form
Proof of Medical Insurance
To upload these items to The Good Game:
- Download The Good Game App (the app is required to upload your physical even if you registered online)
- In the app, log in using the same email and password you used to register (same login whether you registered on the app or the website)
- From the Home Page select your athlete’s Passport. Platinum Passport status is required to participate in this camp. The Passport will guide you through the required steps needed to advance your athlete’s status to Platinum.
- Once Platinum status is complete, your athlete is all set to attend camp!
A physical must be filled out by a physician for your child to participate in camp. A school physical can be used as long as it is completed and dated within a year of the first day of camp. If you DO NOT have access to getting a physical, you may use the “Screening Exam” below to act as a “Physical” and bring that to a doctor and have them fill it out and sign the form.
For support with these steps, please contact support@thegoodgame.com
Can I bring a paper copy of my athlete’s physical exam to check-in instead?
No. Paper copies will not be accepted at check-in. Your athlete’s physical exam certification must be uploaded in the Good Game app prior to check-in in order to be admitted to camp.
